In the diagram there are five swim lanes: Patient, Front Office/Billing Office, Nurse and Physician, Admission/Radiology/Laboratory/Pharmacy. Any process that falls on a particular swim lane is carried out by that swim lane. For instance "Make An Appointment" falls on the the PATIENT swim lane, so is carried by the PATIENT. But a process that falls between two swim lanes are carried out by either swim lanes. For instance "Enter Results, Prescription, Referral, etc into Mega-Net HospitalPro" process falls between NURSE and PHYSICIAN lanes.
